Overall Objectives:

  1. Identify optimal patient care initiatives based on Heart Failure Guidelines.
  2. Improve the knowledge base of the Advanced Heart Failure team by analyzing new research in the field of Advanced Heart Failure treatments. 
  3. Demonstrate improved competency in the identification and care of the Advanced Heart Failure patient. 
  4. Translate medical knowledge to optimal patient care at the bedside.

All Aurora Health Care Providers - Cardiology, Transplant Cardiology, Advanced Heart Failure Physicians, Internal Medicine, Nurse Practitioners, Pharmacist, RN Transplant and Heart Failure Coordinators
