Teaching Primary Palliative Care Stage C

In the final stage of Teaching Primary Palliative Care, you will revisit the educational tools provied in Stages A and B, then reflect on this activity and provide insights on how you practiced the tools you learned in the live coursework, and how you will continue to improve by:

  • Providing direct or consultative care to patients as part of the activity.
  • Implementing the activity's interventions (the changes designed to improve care) and reflect on the tools you selected, to see if they are the correct tools to make a change.

If you feel you have not accomplished your goal and would like further guidance or practice, Dr. Jessick has offered private coaching consultations for individuals who would like additional practice. You will be asked at the end of the self-assessment if you would like more coaching.

"Active participation" means the practitioner must:

  • Provide direct or consultative care to patients as part of the QI project.
  • Implement the project’s interventions (the changes designed to improve care) and reflect on the tools you selected to see if they are the correct tools to make a change.
  • Collect, submit, and review data in keeping with the project’s measurement plan with two linked cycles of data review.
  • Collaborate by actively participating in the live session and subsequent shadowing.
