Cardiac Care Pathways: Heart Failure, Atrial Fibrillation
The Cardiac Care Pathway process reflects a collaborative effort by Aurora primary care providers and cardiovascular specialists to assemble and condense the most current guidelines, best practices and assessment tools to manage patients.This course will walk you through two important pathways for Atrial Fibrillation and Heart Failure by assisting you in effective and efficient management of patients as well as give you resources within Aurora Health Care.
The course presents the Heart Failure Pathway content followed by a short quiz, and then the Atrial Fibrillation Pathway content followed by a short quiz. You must pass both quizzes with 100% to receive credit. At the end of each slide presentation, you will need to close the window to return back to the main course content.

- Identify high risk patients
- Identify treatment algorithms
- Identify patients who may benefit from advanced treatment options
- Reduce re-admissions for HF and AF.
