Overall Objectives:
- Identify optimal patient care initiatives based on Heart Failure Guidelines.
- Improve the knowledge base of the Advanced Heart Failure team by analyzing new research in the field of Advanced Heart Failure treatments.
- Demonstrate improved competency in the identification and care of the Advanced Heart Failure patient.
- Translate medical knowledge to optimal patient care at the bedside.

All Aurora Health Care Providers - Cardiology, Transplant Cardiology, Advanced Heart Failure Physicians, Internal Medicine, Nurse Practitioners, Pharmacist, RN Transplant and Heart Failure Coordinators
